While some hospital admissions are unexpected, if you have a scheduled procedure or admission, please be sure to bring the following with you:

  • A photo ID
  • A list of all current medications
  • Proof of insurance
  • Names and phone numbers of emergency contacts

In accordance with guidelines set by the U.S. Department of Health and Human Services, all hospitals are required to provide a list of standard charges for their services. To access our list of charges, click here.
