Dr. Natalie Hobdy will be departing DHCHD and will cease providing services at our facilities as of January 30, 2023.
To transfer to one of the other providers, please complete a new patient packet and return to that provider’s receptionist once completed. You can find new patient packets on the hospital website under each doctor’s tab or request one from the receptionists in person.
Medical records can be transferred to another provider upon request via the completion of an authorization with the receptionists or the Medical Records department in person. Please call 806-244-5668 for any questions or concerns.
