Coon Memorial Swing Bed Rehabilitation
The Swing Bed Rehabilitation Program at Coon Memorial Hospital is a bridge between hospital and home. Offering services including physical, speech and occupation therapy, as well as skilled nursing and discharge planning, our interdisciplinary team works with patients to help them gain the independence needed to return home safely. This program eases the transition between hospital and home by providing access to highly trained medical professionals right here in Dalhart.
What is Swing Bed?
We understand there are times when patients no longer require the intense care of hospitalization, but are not quite ready to return home. To serve the needs of smaller communities, Medicare established coverage for Skilled Swing Bed Programs, where a patient can “swing” from acute care status, following illness, injury or surgery, to skilled nursing status without transferring to a skilled nursing facility. Patients also may have been hospitalized away from home, and be transferred to their local hospital for their rehabilitation.

Who qualifies for Swing Bed?
To qualify for Swing Bed, the patient must be hospitalized as an Acute Care Patient for a minimum of three consecutive midnights within a 30-day period. Patients may come from any hospital including ours, as long as they meet the eligibility criteria. Medicare and most insurance providers cover Swing Bed Rehabilitation services under the “Skilled Nursing Facility” benefit. Medicare covers care received in Swing Bed at 100% for the first 20 days, then, at 80% with a 20% co-insurance for the next 80 days. We recommend that patients contact their insurance company for specific coverage prior to making any decisions.
Who can benefit from Swing Bed?
Patients who have recently had surgery, a serious illness or an injury and require time to regain strength and mobility may benefit from Swing Bed. Because the support and encouragement of family and friends is vital to patient recovery, caring for patients close to home allows loved ones to visit often, removing the stress of driving to and from a larger city.
