Public Service Announcement from the Local Health Authority, Dr. Turner:

The rate of COVID infection has increased sharply in the community during the past week. It is absolutely essential that if we want our community to remain open, our schools to remain open, and our businesses to remain open, everyone must be very careful at this time. Please wear a mask anytime you are in public. Please pay attention to social distancing. Please wash your hands frequently. If you have any symptoms at all that could be COVID you should stay at home and contact your healthcare provider. Please do not go to work or to school with symptoms of COVID. These include fever, chills, cough, congestion, sinus pressure, headache, body aches, nausea, vomiting, diarrhea, loss of taste, or smell.

**Please note from Friday, October 9th – Friday, October 16th there will be modified Report Card postings. These postings can be found on the Dalhart Emergency Management Facebook page at: https://www.facebook.com/dalhartoem. This modification is due to the communications member of the OEM team being absent for this time span. Normal Report Card posting will resume on Monday, October 19th.

A long-term care ombudsman helps residents of a nursing facility and residents of an assisted living facility resolve complaints. Help provided by an ombudsman is confidential and free of charge. To speak with an ombudsman, a person may call the toll-free number 1-800-252-2412.

Sec. 181.105. REQUIRED INFORMATION REGARDING CONSUMER ACCESS TO HEALTH RECORDS AND COMPLAINTS. (a) Except as provided by Subsection (b), a covered entity shall prominently post on the entity's Internet website and at any entity facility detailed instructions for a consumer to:
(1) request the consumer's health care records from the entity;
(2) contact the disciplinary or licensing authority for the covered entity; and
(3) file a consumer complaint as described by Section 181.103.
(b) This section does not apply to a covered entity that conducts claims processing, data processing, data analysis, utilization review, or billing on behalf of another covered entity that provides health care services directly to consumers.
