Important patient message regarding prescription refills from Vital Health

We are saddened about the loss of Dr. Nathan Vermedahl in our community, and we understand his death may concern his patients who will need upcoming medication refills. Refills of medications originally prescribed by Dr. Vermedahl at Vital Health can be handled through the High Country Community Rural Health Clinic under the following conditions:
 
• The patient must bring his or her medication bottles needing to be refilled.
• Daily medications will be refilled for 90 days to allow patients time to establish with a new provider.
• Medications that are taken as needed will be prescribed only for 30 days.
• Any Schedule 2 medications that a nurse practitioner may legally prescribe may be refilled on a case-by-case basis with the approval of the clinic director.
• Some medications absolutely require a physician to prescribe and cannot be done through the clinic.
• Refills will ONLY BE DONE ONE TIME BY THE WALK-IN CLINIC. NO EXCEPTIONS.

A long-term care ombudsman helps residents of a nursing facility and residents of an assisted living facility resolve complaints. Help provided by an ombudsman is confidential and free of charge. To speak with an ombudsman, a person may call the toll-free number 1-800-252-2412.

Sec. 181.105. REQUIRED INFORMATION REGARDING CONSUMER ACCESS TO HEALTH RECORDS AND COMPLAINTS. (a) Except as provided by Subsection (b), a covered entity shall prominently post on the entity's Internet website and at any entity facility detailed instructions for a consumer to:
(1) request the consumer's health care records from the entity;
(2) contact the disciplinary or licensing authority for the covered entity; and
(3) file a consumer complaint as described by Section 181.103.
(b) This section does not apply to a covered entity that conducts claims processing, data processing, data analysis, utilization review, or billing on behalf of another covered entity that provides health care services directly to consumers.
