Community members will drive through Legacy Assisted Living parking lot to get a lab voucher & vendor bag on Thursday, October 7th, from 7am – 9am. Lab Testing with a voucher will be open on October 13th, 20th, & 27th from 8am – 10am at Coon Memorial Lab. Labs include: CBC, CMP, LIPID,HGBA1C.
- Step 1: Get Lab Voucher: On Thursday, October 7th from 7 – 9am drive through at Legacy Assisted Living located at 301 E Texas Blvd Dalhart, TX 79022. Follow the signs for drive through directions.
- Step 2: Get Lab Testing: Lab testing will be open on October 13th, 20th, & 27th from 8am – 10am at Coon Memorial Lab located at 1411 Denver Ave. Please fill out the pre-admit form before arrival. This form can be found in your packet you will get on Oct 7th. After checking in at Admissions you will go to the lab for your testing. Ring the doorbell. Hand the lab your voucher. Get your blood drawn.
- Step 3: Get Results: Your results will be emailed to you on the patient portal. Admissions will send an auto-generated email to your email at registration. Follow the prompts to get set up. Results will appear in your portal within 48 hours. The portal can be found at: https://www.thrivepatientportal.com/CoonMemorialHospital/. If you do not get the automated email please call 806.244.9258.

- Must be 18+ years of age.
- Must be fasting, for accurate results.
- Labs to include: CBC, CMP, LIPID, HGBA1C
- It is your responsibility to print out the lab values and to seek care from your personal physician. We will notify you if there is a critical value, but it will not be a provider that reaches out.
