2022 Health Fair

DHCHD will host a drive-through health fair at Legacy Assisted Living on October 27, 2022 from 7:00 – 9:00 AM. Attendees will receive a FREE lab voucher and a vendor bag. Labs include: CBC, CMP, LIPID, and HGBA1C. Testing with a voucher will be open Nov. 2, 9, 16 from 8-11 AM at Coon Memorial Hospital.

Instructions for Health Fair Attendees:

Step 1: Get Lab Voucher: On Thursday, Oct. 27th from 7- 9am drive-through at Legacy Assisted Living located at 301 E Texas Blvd Dalhart, TX 79022. Follow the signs for drive-through directions.

Step 2: Get Lab Testing: Lab testing will be open on November 2nd, 9th, & 16th from 8am – 11am, at Coon Memorial Lab located at 1411 Denver Ave. Please fill out the pre-admit form before arrival. This form can be found in your packet. After checking in at Admissions you will go to the lab for your testing. Ring the doorbell. Hand the lab your voucher. Get your blood drawn.

Step 3: Get Results: Your results will be emailed to you on the patient portal. Admissions will send an auto-generated email to your email at registration. Follow the prompts to get set up. Results will appear in your
portal within 24 hours. The portal can be found at: https://coonmemportal.meditech.cloud/ If you do not get the automated email please call 806.244.9258.
