Home Health & Hospice

DHCHD offers both home health and hospice services. Coon Memorial Home Health provides nursing services, therapy services, and lifestyle services to help you stabilize your health and continue to be an active part of your community. Home health can help if you have had:
- A recent hospital stay
- A newly diagnosed condition
- Changes to medication
- Trouble with falls or weakness
- Difficulties with daily activities
Coon Memorial Home Health Services also provides in-home therapy as well as home health aides to assist you with activities of daily living. Home health nurses can help you with:
- Wound care
- Catheter and ostomy care
- Pain management
In addition, Coon Memorial Home Health nurses can help you manage your condition and learn more about your diagnosis and medications.
Hospice services can help patients and family members by providing nursing care as well as emotional support. Hospice services include:
- Pain management
- Medication management
- Social services and spiritual counseling
Our hospice program also provides needed medication, medical equipment, and medical supplies. We also offer bereavement counseling and supportive counseling for survivors after the death of a loved one.
In most cases, Medicare and Medicaid will cover all costs associated with home health and hospice services. Most commercial insurance will also cover this care.
